    • 1) A copy of your Social Security Card, or a copy of your USA Birth Certificate, Resident Alien Card, Employment Authorization Card or United States Passport.
      2) A copy of a photo ID, such as a Driver's License, or State ID, or Passport.
      3) A current and up to date resume.
      4) Copies of your Professional Licenses or Diplomas, (if applicable).
      5) Also, copies of your DEA, Controlled Substances Licenses and liability insurance, (if applicable).
      6) Please note: for Hygienists, please include a copy of your Nitrous Oxide and/or Local Anesthesia certificate.
      7) A copy of your Letter of Recommendation from a current or previous employer or supervisor / instructor.
      8) Lastly, select the applicable DAPS Agreement from the list of 3 below: print, complete and sign and upload with the other documents.
